For claim processing, you can fax our billing department directly at 503-941-3778. If a facility or insurance provider needs to send or discuss claims, our PO Box is 23577, Tigard, OR 97281.

Medical records

If you're a patient, you can view your records and test results online in your ZoomCare patient portal.
